Sheffield Primary Care Trust is using Warfarin as the basis for the 2008/09 Clinical Audit element of the  Pharmacy Contractual Framework. The audit pack has been sent to all contractors but, for convenience, can be accessed and downloaded here .

This supports the guidance issued by the National Patient Safety Agency on anticoagulants.
